I recognize that there are certain inherent risks associated all physical activities and exercises. I assume all risks of injury, illness, or death and hold LEFIT MUAY THAI LLP and its employees harmless in the event of any of the mentioned occurrences. If applicable, I have obtained medical clearances needed to use the equipment and/or start the exercise routine. LEFIT MUAY THAI LLP shall not be responsible for any loss of my personal property. This waiver and release of liability includes, without limitation, all injuries that may occur as a result of

(a) my use of all amenities and equipment in the facility and my participation in any activity, class, program, personal training or instruction, (b) the sudden and unforeseen malfunctioning of any equipment and (c) our instruction, training, and supervision. • I acknowledge that I have carefully read this "Waiver and Release" and fully understand that it is a release of liability.

I agree to indemnify and defend LEFIT MUAY THAI LLP against all claims, causes of action, damages, judgments, costs or expenses, including attorney fees and other litigation costs, which may in any way arise from my or my family's use of or presence upon the facilities of LEFIT MUAY THAI LLP.

I also consent to my image being taken, used and/or reproduced in any format. I understand that any intellectual property, including copyright and image rights, which arises in the visual image(s) belong to LEFIT MUAY THAI LLP.
